****Must be a computer (MacOS or Windows)

1) Make sure you have Adobe Acrobat Reader installed

3) Open the file on your computer using Adobe Acrobat Reader.

4) Fill in the form and then save it.

5) Email the filled-out form to:

You can also print and fill in the form by hand.
